Default remove caliper. pull to remove rotor.

Default There are too large hex head bolts... I forget the size... a BFH might come in handy too.

Default just the ones holding the caliper on, iirc. maybe 18mm??? the lug bolts hold teh rotor on.

Default 18mm bolts and they are usually very tight, i.e. you will need a breaker bar and some muscle

Default by hex-head you mean allen bolts? pretty sure they are standard bolts (use std socket).
