Can 2019 Q7 connect wirelessly to Apple CarPlay or do I have to use a lightning cable?

No. The 2020 model of Q7 is wireless. The 2019 uses USB.

I have a CarlinKit wireless CarPlay adapter and don't recommend it - you'll experience lag on audio playback and phone calls. Carplay2air seems a very similar product if not a re-brand as the update IP is the same as mine. Updates haven't fixed these problems. Just use wired.

I have a '19 Q7 and got a Carlinkit 2.0. It connects CarPlay wirelessly and does a very good job. Every once in a while it has a little trouble connecting, but a quick reboot of the MMI takes care of that. Otherwise it is seemless.

I have the previous version (CarlinKit 1.0) so maybe they fixed whatever it was in the new version.

If that's the case, they should provide an upgrade path for you IMO.

The new firmware was supposed to fix these issues, but it didn't. I may reach out to the vendor for 2.0 discount /upgrade.

I purchased and installed Carlink 2.0 in my 2019 Q7 today. 30 seconds for set up. Works flawlessly.

Bought and installed Carlink 2.0 and it has worked about 90% of the time in terms of 'connecting' automagically to my 2019 Q7. However, the last firmware update has fixed the connectivity issue and the Carlin 2.0 was able to connect 100% of the time.
However, with that said, there is still a bit of lag such as music playback control (when I clicked on play, the music won't play until about 2 seconds later). also, there is a slight delay to the phone call as well (probably the same issue as the bluetooth audio). other than these small hiccups, I'd recommend the adapter.
