It's pretty straight-forward. It goes around the hitch shaft, and that little 'hook' part grabs the receiver hitch lip, then you tighten the two nuts down, and it holds it all together so it doesn't rattle.
The cheaper ones on Amazon (little bent steel flat bar and u-bolt) work just fine, too..

The plastic side-trim... you have to remove a few bits before it will come out. search YT for Q7 amplifier removal and you'll find several how-to vids on getting that dismantled.

When I was in Germany for work earlier in October, there were some sweet mirror extensions on a euro SUV that I saw while on the autobahn, I need to try and track them down...They would totally have worked with our dumbo ear mirrors...
