First, you need to fill up your tank completely, set your odometer to zero, drive the Q5 as you normally do until you have about a quarter tank left, fill up the tank completely, record the number of gallons put in at the pump, divide the odometer reading by those number of gallons. That is the real world actual gas mileage. Guessing at MPG looking at the gas gauge levels is not accurate at all. You live in cold county you are burning the "winter blend" of gas, that alone could drop your mileage by 2-3 MPG. Also cold temperatures make an engine run rich, you burn more gas.

Thanks for all of the feedback and suggestions. I did log the info you suggested as well as my most recent fill up. In summary, my calculations are still showing really low mpg, even on my 3rd fill up where my drives were like 80% highway and 20% city. This can't be good. I'm scheduling a visit to the dealership so they can check this out. I also plan on showing them my recent fuel stats as well shown below (I've also started using Fuelly.com). Do you agree that something is wrong? Any additional thoughts?

It does sound low, but it would probably help to better understand what "hwy" and "city" exactly means in your case... what is your cruising speed on the hwy? Do you stay at this speed almost all the time or do you do a lot of speed ups / slow downs? Is your terrain flat or hilly? And in the city, is there a lot of bumper to bumper traffic that you encounter? Are you stopped/sitting/idling a lot?
City for me for the most part, has been regular, stop signs and red lights, I wouldn't say it's bumper to bumper. I've driven only 1 day, for about 10 miles with bumper to bumper traffic in NYC and that's part of the 20% hwy I did as described in my last 3/20/18 fuel up. I've tried using the Auto start/stop feature to see if that helped but I can't really tell.

Hwy for me is cruising mostly between 50mph-70mph (flat roads on NJ Turnpike or Garden State Parkway), hitting 70mph-80mph a few times, but only to pass. It's not like I'm out there racing

On my 16 Q5 3.0T, if I drove in stop/go city traffic and short trips, 12mpg is normal. I got 28mpg on pure highway driving. Overall about 22mpg for my style of driving.

You are opening your wallet wide up for the dealer, without confirming your car indeed has a problem.
Local/city driving has lots of variables, not a good way to judge MPG. Only pure highway mpg is valid for comparison.
How about driving your car on the highway for 50 miles and read the MPG from dashboard? If you are getting more than 25, your car is normal.
Then you can add some fuel injector cleaner to see if there is an improvement.
10mpg during winter short trips is normal for a 6 cylinder.

Only have a Q5 2.0Tfsi. Not sure what applies to 3.0T. Because of my high fuel consumption I am planning to do the following: (I am averaging 12.5l/100 km instead of usual reported 9l/100km)
1. Change the engine coolant temperature sender. (cheap).
2. Clean MAF sensor with electrical contact cleaner (same procedure of removing air filter) - Done yesterday.
3. Check for intercooler for leaks or upgrade intercooler to a full metallic higher capacity intercooler. (bought the upgraded intercooler but did not have time to do the work)
4. check or change turbo boost (and vacuum) lines.
(sparks are new, coils dont seem to have a problem since engine is smooth).
