Why is my car jerking when I drive?

I have an 87 5KSQ. Lately, I have noticed that when I drive it will jerk, sometimes accompanied by the feeling that I am driving straight into the wind (On extremely non-windy days). It all goes away when I push the clutch pedal. Is this simply my clutch slipping/grabbing, or should I look a little more carefully into it?

BTW, I already know my clutch needs replacing.

It's not related to the way the engine is running? Unless there's oil or something on the clutch disc, or the pressure plate is weak.
