So, these salesmen sent you this message directing you on how to answer the post-delivery survey?<p>I believe I would point that out to the surveyer before the questions began.<p>Another option- call that number and ask the salesmen if they think you are an idiot that cannot answer the survey without cheat notes.<p>Have fun!

A few people have known about the survey in advance and have used it as a tool to get a better deal on their car or to get a 'free' CD player or something like that.<p>It sounds like you have already taken delivery of your car, so it may be harder for you to do this, but it's worth a shot.<p>Matt

And yes, the dealer attaches a "answer this way" sheet to the receipt. I think it is just a tactless way of saying "if there is anything we could have done better, please tell us". I think the psychology of it makes people more likely to answer "excellent" than "good", which is the point. I don't think there is anything sleazy going on, if that is your concern. Just hopefully they haven't given you any reason that those answers wouldn't be the correct one.
