The following info describes what I found in my 98.5 A4 1.8T, with Concert/non-Bose (and Panasonic head unit) radio. Your findings/results may differ from mine (a.k.a., "anything you do is not my fault/use this information at your own risk!").
Also note that there are other items in the archives describing door removal, rear deck removal, the telephone speaker being integrated into the driver's side front speaker, and so forth. The information I am providing here is meant to "fill in the gaps" not expressed in those other postings (I did a lot of searching in the archives trying to find this information, but please feel free to point out any other articles in the archives that I may have missed).
Mounting info for front speakers:
Note that the front speakers in the car must be removed/installed from the back, so refer to the archives (e.g, the article entitled "Replacing Front Speakers") for more info. And one more thing - note that the speaker grilles are not removable from the fronts of the door panels.
The tweeters are mounted in special 1 ¾ inch plastic keyed brackets that fit into the back of the integrated inside door opener/tweeter assemblies.
The woofers are each mounted to three plastic studs that are arranged in an isosceles triangle (in other words, the studs are not all the same distance apart from each other). The mounting measurements (from hole center to hole center) are as follows:
Bottom left to bottom right stud: 3 ½ inches
Bottom left to upper (topmost) stud: 4 5/8 inches
Bottom right to upper (topmost) stud: 4 5/8 inches
Also note that a 4 inch diameter speaker hole is molded into the door panel, such that it probably doesn't make sense to try and fit woofers larger than 4 inches unless you want to do a lot of reworking/modification of the door panel and speaker mounting points.
I found 4-ohm Blaupunkt woofers and tweeters in the front doors of my car. Other people have mentioned finding Nokia speakers (perhaps these cars were slightly older than mine; I'm not sure).
Mounting info for rear speakers:
The rear speakers are indeed self-amplified, and are mounted from above. Once again, the grilles are not removable from the top; instead, refer to the archives (e.g., the article entitled "Rear Deck (removing)") for more info on how to remove the entire rear deck. When removing my rear deck, I found three "push-in" locks along the leading edge (no screws found/needed to be removed to get my deck out; just the connector for the third brake light), which I carefully collapsed with long-nosed pliers in order to get them up through their holes.
As with the front speakers, the rear speaker mountings are not "standard." In particular, the holes cut into the rear deck are square (not round), and each side of the square measures 4 ½ inches. Most 6 ½ inch speakers will probably require some cutting of the rear deck in order to fit properly (BTW, the stock speakers in my car measured about 6 inches in diameter, with about 1 inch of height from the base of the top-deck speaker mounting to the top of the "tweeter" that's mounted in the center of each speaker).
In addition, each rear speaker is mounted to the deck from the top using two screws. The two screw mounting holes are 7 inches apart on center, and are placed in a fore-aft configuration (as opposed to side-to-side). However, there is a slight rise in the rear deck metal (perhaps 3/16 of an inch) for the screw mounts; this means that most (if not all) replacement speakers would not mount flush to the rear deck, so plan on some modification here if you want a solid speaker mounting.
I found 4-ohm Nokia [self-amplified] speakers in the rear deck of my car. Wiring from the front of the car was connected to the left rear speaker (which is where the amplifier appears to be), and wires for the right rear speaker were routed from the left rear speaker and under the leading lip of the rear deck.
